Overview

**Magrolimab + azacitidine** is a combination therapy consisting of magrolimab, a first-in-class humanized monoclonal antibody targeting cluster of differentiation 47 (CD47), and azacitidine, a hypomethylating agent. Magrolimab blocks the CD47 "don't eat me" signal on cancer cells, promoting macrophage-mediated phagocytosis. Azacitidine upregulates prophagocytic "eat-me" signals, increasing the ability of magrolimab to induce tumor cell clearance. This combination is being investigated for the treatment of higher-risk myelodysplastic syndromes (MDS) and acute myeloid leukemia (AML), especially in patients with TP53 mutations who have poor prognosis and limited treatment options. Early-phase studies showed promising safety and remission rates, but recent randomized phase III trials resulted in early termination due to lack of efficacy in AML and higher-risk MDS[1][2][4][5][6][9].
