MAHDL (also known as AR-601) is an investigational synthetic high-density lipoprotein (HDL) mimetic developed by Aridis Pharmaceuticals for the treatment of sepsis and septic shock. It is designed to mimic the pleiotropic protective functions of natural HDL, which are typically compromised or depleted during severe systemic infections. MAHDL acts by directly binding and neutralizing bacterial toxins, including lipopolysaccharide (LPS) from Gram-negative bacteria and lipoteichoic acid (LTA) from Gram-positive bacteria, thereby preventing the systemic inflammatory cascade and subsequent multi-organ failure. Additionally, MAHDL interacts with scavenger receptor class B member 1 (SR-BI) to exert anti-inflammatory, antioxidant, and anti-apoptotic effects, helping to maintain endothelial integrity and protect against organ damage, such as acute kidney injury. The compound is currently in preclinical development and has shown potential in reducing mortality and protecting organ function in animal models of sepsis.
