Mangafodipir is a small molecule, organ-specific paramagnetic contrast agent used in magnetic resonance imaging (MRI), primarily for the detection of lesions in the liver and pancreas. It consists of manganese(II) ions chelated by fodipir (dipyridoxyl diphosphate, DPDP). After intravenous administration, the chelate slowly dissociates; manganese is selectively taken up by hepatocytes, while fodipir is distributed to extracellular fluid and eliminated via urine. The uptake of manganese by normal liver tissue increases signal intensity on T1-weighted MRI images, enhancing contrast between healthy and abnormal tissues—since metastatic or cancerous tissues absorb less manganese. Mangafodipir was marketed as Teslascan but has been withdrawn from both US and European markets for commercial reasons. Beyond its use as an imaging agent, it also exhibits mitochondrial superoxide dismutase mimetic activity and has been investigated as an adjunct in chemotherapy and coronary interventions[1][7][5].
