Mannan-conjugated allergoid Dermatophagoides pteronyssinus + Dermatophagoides farinae is a parenteral allergen immunotherapy (AIT) vaccine developed by Inmunotek S.L. for the treatment of house dust mite (HDM)-induced allergic respiratory diseases. The vaccine consists of polymerized allergoids (allergens chemically modified to reduce IgE reactivity while preserving immunogenicity) derived from the two most common HDM species, *Dermatophagoides pteronyssinus* and *Dermatophagoides farinae*, which are covalently conjugated to mannan. Mannan, a polysaccharide derived from yeast, acts as a natural adjuvant by targeting C-type lectin receptors (such as the mannose receptor/CD206) on dendritic cells. This targeting facilitates the efficient uptake and processing of the allergens, promoting a shift from a Th2-mediated allergic response toward a Th1 and regulatory T cell (Treg) profile, thereby inducing long-term immunological tolerance. The product is currently in Phase III clinical development for patients with moderate-to-severe allergic rhinitis or rhinoconjunctivitis, including those with comorbid mild-to-moderate allergic asthma.
