Mapatumumab is a fully human monoclonal antibody that selectively binds to and activates the tumor necrosis factor-related apoptosis-inducing ligand receptor 1 (TRAIL-R1), also known as death receptor 4 (DR4). By agonizing TRAIL-R1, it induces apoptosis in cancer cells and reduces tumor growth. It was developed primarily for cancer treatment indications including non-Hodgkin's lymphoma, non-small-cell lung cancer (NSCLC), colorectal cancer, hepatocellular carcinoma (liver cancer), and multiple myeloma. Mapatumumab has been investigated both as monotherapy and in combination with chemotherapies such as paclitaxel/carboplatin or gemcitabine/cisplatin. Despite early phase clinical trials showing safety and some disease stabilization or partial responses in advanced solid tumors and lymphomas, later phase II trials failed to demonstrate significant efficacy improvements over standard treatments. Development for several cancers has been discontinued due to lack of sufficient clinical benefit.
