Marifupeptide is a synthetic neuroprotective peptide developed by the Second Military Medical University (Naval Medical University) in China. It is designed for the treatment of acute ischemic stroke, specifically targeting the excitotoxic cascade that leads to neuronal death following a stroke. The peptide acts by disrupting the interaction between postsynaptic density protein 95 (PSD-95) and neuronal nitric oxide synthase (nNOS). By uncoupling these proteins, marifupeptide prevents the overproduction of nitric oxide and other toxic downstream signals typically triggered by excessive NMDA receptor activation during ischemia. This neuroprotective strategy aims to salvage the ischemic penumbra and improve clinical outcomes. As of 2017, the drug was undergoing Phase 1 clinical evaluation in healthy volunteers to assess its safety, tolerability, and pharmacokinetics.
