Overview

Marizomib + vorinostat is an investigational combination therapy for cancer, particularly studied in hematologic malignancies such as multiple myeloma. Marizomib (NPI-0052, salinosporamide A) is a structurally unique β-lactone–γ-lactam proteasome inhibitor that irreversibly inhibits all three proteolytic activities of the 20S proteasome, leading to accumulation of misfolded proteins and apoptosis in cancer cells. Vorinostat (suberoylanilide hydroxamic acid, SAHA) is a histone deacetylase (HDAC) inhibitor that blocks class I and II HDAC enzymes, resulting in increased acetylation of histones and other proteins. This leads to altered gene expression, cell cycle arrest, apoptosis, inhibition of angiogenesis, and downregulation of immunosuppressive interleukins. The combination has shown synergistic anti-tumor activity in preclinical models and early-phase clinical trials for relapsed/refractory multiple myeloma[4][5][6].
