Overview

MART-1 reactive CD8+ peripheral blood lymphocytes (PBL) is an autologous adoptive cell therapy developed by the National Cancer Institute (NCI) for the treatment of metastatic melanoma. The therapy involves the isolation and ex vivo expansion of naturally occurring CD8+ T cells from a patient's peripheral blood that exhibit reactivity against the Melanoma Antigen Recognized by T cells 1 (MART-1), a protein highly expressed in most melanomas. Following a lymphodepleting preparative regimen of cyclophosphamide and fludarabine, these MART-1-specific T cells are re-infused into the patient, often accompanied by high-dose aldesleukin (IL-2) to promote T-cell persistence and activity. The mechanism of action relies on the T-cell receptor (TCR) of the infused CD8+ cells recognizing MART-1 peptides presented by HLA-A2 on the surface of melanoma cells, leading to targeted tumor cell lysis.
