MAX-8 is a small-molecule radioligand therapy (RLT) developed by Guangzhou Zaiji Medical Technology (Maxinovel). It is designed to target Fibroblast Activation Protein (FAP), a cell-surface protease that is highly expressed on cancer-associated fibroblasts (CAFs) within the stroma of various solid tumors, while maintaining low expression in most normal adult tissues. By specifically binding to FAP, MAX-8 delivers a therapeutic radioactive isotope directly to the tumor microenvironment, facilitating targeted irradiation that induces DNA damage and subsequent cell death in both tumor cells and the supporting stroma. The drug is currently being evaluated in Phase 1/2 clinical trials for the treatment of advanced solid tumors.
