Overview

MB0109 is a first-in-class monoclonal antibody targeting the C1q component of the classical complement pathway, currently under development by Montis Biosciences. Unlike many complement inhibitors that target the globular heads of C1q, MB0109 uniquely binds to specific epitopes on the tail region. This binding enables a dual therapeutic effect: it inhibits the classical complement cascade and simultaneously prevents C1q-driven activation of macrophages and microglia. The drug is primarily aimed at treating demyelinating autoimmune neuropathies such as Guillain-Barré Syndrome (GBS) and Chronic Inflammatory Demyelinating Polyradiculoneuropathy (CIDP), with additional potential applications in high-C1q subsets of Multiple Sclerosis and Vascular Dementia. As of early 2026, MB0109 is in preclinical development, with Phase 1 clinical trials projected to commence in mid-2027.
