A membrane BCMA (mBCMA)-CD3 bispecific antibody is a T-cell engaging immunotherapy designed to treat B-cell malignancies, most notably multiple myeloma. These molecules possess dual binding specificity: one arm binds to the B-cell maturation antigen (BCMA), which is highly and selectively expressed on the surface of malignant plasma cells, while the other arm binds to the CD3 epsilon subunit of the T-cell receptor complex. By cross-linking these two targets, the antibody facilitates the formation of a cytolytic synapse between cytotoxic T cells and myeloma cells, leading to T-cell activation, degranulation, and the subsequent perforin-mediated lysis of the tumor cells independent of MHC-I expression. The "mBCMA" designation often refers to the membrane-bound form of the receptor, distinguishing it from the soluble form (sBCMA) that can act as a decoy. Several agents in this class, such as teclistamab and elranatamab, have received regulatory approval, while others remain in clinical development.
