Overview

MBF-118 is a small molecule, orally administered benzamide derivative developed by Medibiofarma. It acts as a partial agonist of the peroxisome proliferator-activated receptor gamma (PPARγ), covalently binding to this nuclear receptor with high selectivity and limited off-target effects. PPARγ plays a key role in regulating inflammation, fibrosis, and immunity. By partially activating PPARγ, MBF-118 exerts anti-inflammatory and antifibrotic effects while minimizing side effects associated with full agonists. The drug is gut-enriched with limited systemic exposure, making it particularly suitable for gastrointestinal diseases such as Crohn’s disease—especially in patients with fibrostenotic complications. Preclinical studies have shown efficacy in models of inflammation and fibrosis; clinical trials have demonstrated good safety and tolerability profiles. Development has focused on Crohn’s disease (Phase II), while development for diabetes mellitus and idiopathic pulmonary fibrosis has been discontinued[1][2][6][8].
