mbIL21-expanded donor-derived natural killer cells is an adoptive cell therapy developed by the Federal Research Institute of Pediatric Hematology, Oncology and Immunology. The therapy involves the ex vivo expansion of natural killer (NK) cells obtained from a donor (haploidentical or matched-related) using a feeder system expressing membrane-bound interleukin-21 (mbIL21). This expansion process is designed to significantly increase the number and cytotoxic potential of the NK cells. The cells are administered to pediatric and young adult patients with chemorefractory or minimal residual disease (MRD) positive acute leukemias, including acute myeloid leukemia (AML), acute T-lymphoblastic leukemia (T-ALL), and mixed phenotype acute leukemia (MPAL), in conjunction with hematopoietic stem cell transplantation (HSCT). The goal of the therapy is to provide a potent graft-versus-leukemia effect to eradicate residual leukemic cells and prevent disease relapse post-transplant.
