MCD-18STA-18 is a novel small-molecule covalent inhibitor of malonyl-CoA decarboxylase (MLYCD) currently under preclinical investigation for the treatment of glioblastoma (GBM). MLYCD is a key enzyme that regulates the balance between fatty acid synthesis and oxidation by converting malonyl-CoA into acetyl-CoA. By inhibiting MLYCD, MCD-18STA-18 increases intracellular levels of malonyl-CoA, which in turn inhibits carnitine palmitoyltransferase-1 (CPT1), the rate-limiting step of mitochondrial fatty acid oxidation (FAO). This metabolic disruption leads to energy depletion and induces apoptosis in glioma cells, which are known to rely on FAO for survival and adaptation to hypoxic environments.
