Overview

**MCD-386** is a highly selective small molecule agonist of the M1 muscarinic acetylcholine receptor developed by Mithridion for central nervous system disorders. It mimics acetylcholine to restore cognition, memory, executive function, and attention impaired by cholinergic deficits, with potential disease-modifying effects such as reducing amyloid-beta production in Alzheimer's models and activating hippocampal signaling pathways. The controlled-release formulation (MCD-386CR) received FDA Orphan Drug Designation for progressive supranuclear palsy (PSP) and has completed Phase 1 trials in healthy volunteers, demonstrating good tolerability and sustained pharmacokinetics; it shows promise for Alzheimer's disease, schizophrenia, and other cognitive impairments.
