mCD19-CAR-CD28-CD3-zeta allogeneic PBL is an investigational allogeneic chimeric antigen receptor (CAR) T-cell therapy developed by the National Cancer Institute (NCI). The therapy consists of peripheral blood lymphocytes (PBL) derived from an HLA-haploidentical donor, which are genetically modified using a retroviral vector to express a CAR targeting the CD19 antigen. This CAR construct includes a CD28 costimulatory domain and a CD3-zeta signaling domain to promote T-cell activation and expansion. It is being evaluated in Phase I clinical trials for patients with high-risk CD19-expressing hematologic malignancies, such as B-cell leukemias and lymphomas, specifically in combination with haploidentical hematopoietic cell transplantation (HCT) and post-transplantation cyclophosphamide (PTCy) to enhance the graft-versus-leukemia effect while managing graft-versus-host disease.
