MD-0727 (also known as IW-0727) is a novel, non-absorbed, high-capacity, cross-linked polymeric bile acid sequestrant that was developed by Ironwood Pharmaceuticals (formerly Microbia) for the treatment of primary hypercholesterolemia. The compound functions by binding bile acids within the gastrointestinal tract, thereby preventing their reabsorption in the ileum and facilitating their fecal excretion. This process leads to a reduction in the bile acid pool, which prompts the liver to upregulate the conversion of endogenous cholesterol into new bile acids, ultimately resulting in a significant decrease in circulating low-density lipoprotein cholesterol (LDL-C) levels. Although MD-0727 demonstrated efficacy and safety in Phase 2 clinical trials, Ironwood Pharmaceuticals discontinued its development to focus resources on its gastrointestinal lead, linaclotide.
