mdCD7-CAR-NK-92MI is a monoclonal, bivalent chimeric antigen receptor (CAR) natural killer (NK) cell therapy targeting the CD7 antigen. Developed for the treatment of T-cell acute lymphoblastic leukemia (T-ALL), it utilizes the NK-92MI cell line, which is an interleukin-2 (IL-2) independent derivative of the NK-92 cell line. The CAR construct features a bivalent CD7-specific nanobody (VHH6-VHH6) linked to CD28 and 4-1BB costimulatory domains and a CD3ζ signaling domain. By targeting CD7, which is highly expressed in T-ALL but absent on a subset of normal T cells, and using NK cells as effectors, this therapy aims to avoid the fratricide common in CD7-targeted CAR-T therapies while providing a potent, off-the-shelf cytotoxic response. The 'md' prefix indicates a monoclonal derivative of the bivalent (d) CD7-CAR construct.
