Overview

MDH-111 is an inhaled microbial extracellular vesicle (EV)-based therapeutic derived from the commensal bacterium *Micrococcus luteus*. Developed by MD Healthcare, MDH-111 is designed to treat chronic inflammatory respiratory diseases, including non-eosinophilic (neutrophilic) asthma, chronic obstructive pulmonary disease (COPD), idiopathic pulmonary fibrosis (IPF), cystic fibrosis, anosmia, and chronic rhinosinusitis. The therapeutic works by upregulating hsa-miR-4517 expression in airway epithelial cells, which subsequently suppresses IL-1β production by monocytes, inhibits group 3 innate lymphoid cell (ILC3) activation, and reduces neutrophil recruitment. By suppressing Th17-mediated neutrophilic inflammation and preventing epithelial cell apoptosis, MDH-111 aims to modify the underlying disease course of steroid-resistant respiratory conditions. It is currently in Phase 1 clinical development.
