Overview

MDI-2517 is a potent small molecule inhibitor of plasminogen activator inhibitor 1 (PAI-1), a key regulator of pro-inflammatory and pro-fibrotic processes. Developed by MDI Therapeutics, it is being investigated as an oral therapy for systemic sclerosis (scleroderma) and interstitial lung disease, with additional potential in other chronic fibrotic and inflammatory diseases associated with excessive PAI-1 activity. By inhibiting PAI-1, MDI-2517 aims to reduce pathological fibrosis and inflammation by restoring PAI-1 levels to normal physiological ranges. Preclinical studies have shown that MDI-2517 reduces fibrosis in skin and lungs more effectively than existing treatments such as pirfenidone or mycophenolate mofetil, and at lower doses compared to other PAI-1 inhibitors. The drug is currently in Phase 1 clinical trials evaluating its safety, tolerability, pharmacokinetics, and pharmacodynamics[1][2][4][5][6].
