**Mecasermin rinfabate** is a recombinant human protein complex comprising insulin-like growth factor 1 and insulin-like growth factor-binding protein 3. The IGFBP-3 component binds and buffers IGF-1, prolonging systemic exposure and reducing peak free-IGF-1 activity relative to recombinant IGF-1 alone; IGF-1 then activates the insulin-like growth factor 1 receptor to promote growth, anabolic signaling, and fetal organ development. It was originally developed by Insmed and marketed as IPLEX for pediatric growth failure due to severe primary IGF-1 deficiency or growth hormone gene deletion with neutralizing anti-growth-hormone antibodies, but that short-stature indication was subsequently discontinued. The same asset has subsequently been developed as SHP607, TAK-607, and OHB-607 for complications of extreme prematurity; Oak Hill Bio lists OHB-607, partnered with Chiesi, in an ongoing Phase 2b program.
