Mecobalamin is a synthetic and active form of vitamin B12 that can cross the blood-brain barrier without biotransformation. It is used to treat or prevent vitamin B12 deficiency and its complications[1][2]. As an activated coenzyme form of cobalamin (vitamin B12), mecobalamin participates directly in homocysteine metabolism as a coenzyme for methionine synthase. This enzyme is required for the conversion of homocysteine to methionine in the methylation cycle—an essential process for DNA and protein methylation[5]. Clinically it has been used to treat nutritional diseases related to B12 deficiency as well as neurological disorders such as diabetic neuropathy and peripheral neuropathies[2][4][5]. Evidence suggests it may promote nerve regeneration and provide neuroprotection by antagonizing glutamate-induced neurotoxicity. It has also shown potential analgesic effects in conditions like diabetic neuropathy and neuralgia by improving nerve conduction velocity and reducing pain behaviors[5].
