MEDI1191 is an investigational mRNA-based therapy designed for intratumoral injection. It consists of a lipid nanoparticle encapsulating mRNA encoding the bioactive interleukin-12 (IL-12p70) cytokine, specifically the IL12A (p35) and IL12B (p40) subunits[1][5]. The drug is intended to promote local production of IL-12 within tumors, thereby reprogramming the immunosuppressed tumor microenvironment and enhancing anti-tumor immunity by increasing cytotoxic T-cell recruitment, proliferation, and activation[1][2][6]. Clinical studies have shown that intratumoral administration of MEDI1191 leads to increased levels of IL-12 and IFNγ in serum as well as greater infiltration of activated T cells into tumors[6]. Developed originally by Moderna Therapeutics, it has been evaluated in combination with durvalumab in phase 1 trials for advanced solid tumors including melanoma, breast cancer, colorectal cancer, gastric cancer, pancreatic cancer, head and neck cancers, bladder cancer, non-small cell lung cancer (NSCLC), vulvar cancer and others[2][4][5].
