**MEDI4166** is an investigational, single-molecule peptide-antibody fusion developed by MedImmune and AstraZeneca for type 2 diabetes in overweight or obese patients at elevated cardiovascular risk. It combines a PCSK9-neutralising monoclonal antibody with a DPP-4-resistant GLP-1 receptor agonist peptide fused to the N-terminus of the antibody light chain. The PCSK9 component robustly suppressed free PCSK9 and reduced LDL cholesterol, whereas the engineered GLP-1 component showed inadequate clinical glucose-lowering activity in a Phase 1 study. Development was discontinued after the study demonstrated dose-dependent LDL-cholesterol lowering but no clinically meaningful improvement in postprandial glucose control.
