Overview

MEDI4276 is an investigational biparatopic tetravalent antibody-drug conjugate (ADC) designed to target human epidermal growth receptor 2 (HER2). It consists of a bispecific humanized monoclonal antibody that binds two nonoverlapping epitopes in subdomains 2 and 4 of the HER2 ectodomain, site-specifically conjugated to a tubulysin-based microtubule inhibitor payload via a cleavable linker. Upon binding to HER2-expressing tumor cells, the ADC is internalized and releases its cytotoxic payload, leading to cell death. Developed for the treatment of advanced or metastatic HER2-positive breast cancer and gastric cancer, it demonstrated clinical activity but was discontinued due to intolerable toxicity at higher doses[1][5][10].
