**Medical air equivalent** is an inhaled oxygen-nitrogen gas mixture used as the placebo comparator in the University of Birmingham OM-COPD crossover study, NCT01722370. Delivered from a cylinder at 2 liters per minute, it was formulated to mimic medical air without providing supplemental oxygen enrichment. It has no drug-specific biological target or pharmacologic mechanism and served to maintain blinding while evaluating ambulatory oxygen therapy for exertional hypoxia in chronic obstructive pulmonary disease.
