Overview

This regimen is a multi-agent chemotherapy combination used primarily in the treatment of multiple myeloma, especially in relapsed or refractory cases and for patients undergoing stem cell transplantation. The components are: - **Melphalan** (MEL): an alkylating agent that crosslinks DNA and inhibits DNA and RNA synthesis. - **Carfilzomib** (CFZ): a proteasome inhibitor that blocks the 20S proteasome, leading to accumulation of proteins within cells and apoptosis, particularly in malignant plasma cells[4]. - **Thalidomide** (TD): an immunomodulatory drug with antiangiogenic properties. - **Dexamethasone** (D): a corticosteroid with anti-inflammatory and antineoplastic effects. The PACE backbone includes: - **Cisplatin** - **Doxorubicin** - **Cyclophosphamide** - **Etoposide** This intensive regimen is designed to target multiple pathways involved in myeloma cell survival. It is typically reserved for aggressive disease or as part of conditioning before autologous stem cell transplant.
