MCBC is an intensified conditioning regimen developed for patients with refractory or relapsed acute myeloid leukemia (AML) undergoing allogeneic hematopoietic stem cell transplantation (allo-HSCT). The regimen is a combination of four agents: melphalan, cladribine, busulfan, and cyclophosphamide. It is designed to enhance the anti-leukemic effect and improve outcomes compared to standard conditioning regimens like busulfan and cyclophosphamide (Bu/Cy). Melphalan, busulfan, and cyclophosphamide are alkylating agents that induce DNA damage and cell death in leukemic cells, while cladribine is a purine nucleoside analog that inhibits DNA synthesis and repair, potentially sensitizing cells to the effects of the alkylating agents. This combination is primarily being evaluated by the Institute of Hematology & Blood Diseases Hospital in China to improve recurrence-free survival in high-risk AML patients.
