This therapeutic regimen involves the use of Isolated Hepatic Perfusion (IHP) to deliver high-dose melphalan directly to the liver, followed by systemic administration of the FOLFOX4 regimen (fluorouracil, leucovorin, and oxaliplatin). Developed and studied by the National Cancer Institute (NCI), this approach is designed for patients with unresectable metastatic colorectal cancer confined primarily to the liver. IHP allows for the delivery of chemotherapy concentrations to the liver that would be toxic if given systemically, by surgically isolating the hepatic circulation from the rest of the body's blood supply. Melphalan acts as a DNA alkylating agent, while the FOLFOX4 components provide complementary systemic control of micrometastatic disease.
