Drug intelligence / Profile preview
**Melphalan + ipilimumab + nivolumab** is an investigational combination regimen evaluated for the treatment of unresectable liver-only or liver-dominant metastatic uveal melanoma. The regimen combines regional liver-directed chemotherapy with systemic double immune checkpoint blockade. Melphalan, a small-molecule alkylating agent, is delivered directly to the liver via percutaneous hepatic perfusion (PHP) using the CHEMOSAT Hepatic Delivery System (developed by Delcath Systems), which isolates hepatic blood flow to deliver high-dose chemotherapy while minimizing systemic exposure. This regional therapy is sequenced with systemic administration of the monoclonal antibodies ipilimumab (an anti-CTLA-4 checkpoint inhibitor) and nivolumab (an anti-PD-1 checkpoint inhibitor) to enhance systemic anti-tumor immune responses and address extrahepatic progression. The combination has been evaluated in the investigator-initiated Phase 2 CHOPIN trial (NCT04283890) led by Leiden University Medical Center, demonstrating a significant improvement in progression-free survival compared to PHP alone.
