Menstrual blood-derived mesenchymal stem cell exosomes (MenSC-Exos) are extracellular vesicles (EVs) secreted by mesenchymal stem cells (MSCs) isolated from human menstrual blood. These bilipid membrane vesicles, typically 30–100 nm in diameter, act as a cell-free therapeutic modality by delivering a bioactive cargo of proteins, lipids, mRNAs, and microRNAs (miRNAs) to recipient cells. MenSC-Exos have been investigated for their regenerative, anti-fibrotic, and anti-apoptotic properties in various conditions, including liver cirrhosis, premature ovarian insufficiency (POI), and diabetic wound healing. They modulate key signaling pathways such as SMAD3/AKT/MDM2/P53 and promote angiogenesis through the delivery of factors like angiopoietin-2 and angiogenin. Clinical studies have demonstrated their potential to reduce portal vein pressure and improve liver function when administered via hepatic artery infusion.
