mEphA4-Fc (also known as NUN-004) is a recombinant fusion protein developed by NuNerve Pty. Ltd. for the treatment of amyotrophic lateral sclerosis (ALS). It consists of a modified version of the EphA4 receptor extracellular domain fused to an Fc fragment, engineered to have an extended half-life compared to the native EphA4-Fc construct. The drug acts as an antagonist of the EphA4 (Ephrin type-A receptor 4), a receptor tyrosine kinase that has been identified as a genetic modifier of ALS progression. By inhibiting EphA4 signaling, which is thought to contribute to motor neuron degeneration and limit axonal regeneration, mEphA4-Fc aims to slow the progression of the disease. It has been evaluated in a Phase I/Ib first-in-human study to assess safety, tolerability, and pharmacokinetics in healthy volunteers and patients with ALS.
