Overview

Mepivacaine chlorhydrate, an amide local anesthetic related to lidocaine, is combined with sodium bicarbonate (NaHCO₃) to create a buffered solution. This buffering neutralizes the acidic nature of mepivacaine, reducing pain during infiltration while maintaining anesthetic efficacy and duration. The combination is primarily used for local and regional anesthesia, including nerve blocks, infiltration anesthesia, and epidural anesthesia, as well as for dental procedures and specific blocks like axillary brachial plexus, median, and ulnar nerve blocks. It is also used for local infiltration anesthesia in procedures like ambulatory phlebectomies for varicose vein treatment. Studies show benefits including reduced pain during injection and faster onset of action.
