Mepocilizumab is a humanized monoclonal antibody directed against CD147 (also known as Basigin or EMMPRIN), a transmembrane glycoprotein that is highly expressed on various cell types, including platelets, monocytes, and endothelial cells. Developed by the Second Affiliated Hospital of Air Force Medical University (Tangdu Hospital), mepocilizumab is primarily being investigated for its potential to treat ST-segment elevation myocardial infarction (STEMI). The mechanism of action involves blocking the interaction of CD147 with its ligands, such as Cyclophilin A, thereby inhibiting the induction of matrix metalloproteinases (MMPs) and reducing the recruitment of inflammatory cells to the site of myocardial injury. This intervention is intended to mitigate myocardial ischemia-reperfusion injury, which occurs during primary percutaneous coronary intervention (PCI), ultimately aiming to reduce infarct size and preserve cardiac function.
