Drug intelligence / Profile preview
Meropenem + durlobactam is an investigational combination of the broad-spectrum carbapenem antibiotic meropenem and the novel diazabicyclooctane (DBO) beta-lactamase inhibitor durlobactam (formerly ETX2514). Meropenem acts by binding to and inhibiting penicillin-binding proteins (PBPs), which are essential for bacterial cell wall synthesis. Durlobactam is a potent inhibitor of Ambler class A, C, and D serine beta-lactamases, including Klebsiella pneumoniae carbapenemase (KPC) and OXA-type carbapenemases (such as OXA-48 and OXA-23). By neutralizing these enzymes, durlobactam protects meropenem from hydrolytic degradation, thereby restoring its bactericidal activity against multidrug-resistant (MDR) and carbapenem-resistant Gram-negative pathogens. While durlobactam is FDA-approved in combination with sulbactam (as Xacduro), the combination of meropenem and durlobactam is primarily studied in vitro and in preclinical models to explore its potential against resistant pathogens like Klebsiella pneumoniae, Mycobacterium tuberculosis, and Mycobacterium abscessus.
