Overview

The combination of meropenem and metronidazole is a widely used antibiotic regimen for treating complicated intra-abdominal infections (cIAIs). This combination leverages the complementary antimicrobial spectrum of both drugs to provide comprehensive coverage against aerobic and anaerobic pathogens. ## Pharmacology and Mechanism of Action Meropenem is a carbapenem antibiotic that works by inhibiting bacterial cell wall synthesis. It has broad-spectrum activity against many gram-positive and gram-negative bacteria[6]. The standard adult dosage for intra-abdominal infections is 1 gram administered intravenously every 8 hours[6]. Metronidazole specifically targets anaerobic bacteria and certain protozoal infections. When used in combination with meropenem, it provides enhanced coverage against anaerobic pathogens commonly found in intra-abdominal infections[8]. For bacterial infections, metronidazole is typically administered with a loading dose of 15 mg/kg IV followed by maintenance doses of 7.5 mg/kg IV or orally every 6 hours[8]. ## Clinical Evidence and Applications The combination therapy is primarily indicated for complicated intra-abdominal infections, including peritonitis and appendicitis. Clinical evidence suggests that this combination is effective for treating these conditions, particularly in cases where there is concern for mixed aerobic and anaerobic infections[3]. Recent research has compared the efficacy of meropenem monotherapy versus combination therapies including metronidazole for cIAIs. A meta-analysis of 8 clinical trials found that combined therapy with metronidazole is as effective and safe as carbapenem monotherapy in treating cIAIs[1]. This finding is significant given the rise of Carbapenem-resistant Enterobacteriaceae, as the combination therapy can serve as an alternative treatment approach[1]. ## Alternative Combinations While meropenem plus metronidazole is a standard treatment option, other combinations are also used in clinical practice. For example, novel β-lactam/β-lactamase inhibitors (BL/BLIs) in combination with metronidazole have shown noninferiority compared to meropenem in the treatment of cIAIs[2]. Additionally, in February 2025, the FDA approved EMBLAVEO (aztreonam and avibactam) in combination with metronidazole for treating adults with complicated intra-abdominal infections who have limited or no alternative treatment options[4]. For patients who cannot tolerate carbapenems, alternative regimens such as ceftriaxone (2 g per day for 5 days) in combination with metronidazole are recommended[5]. ## Dosage and Administration For adults with intra-abdominal infections, meropenem is typically administered as 1 g IV every 8 hours[6]. Metronidazole dosing for bacterial infections usually involves a loading dose of 15 mg/kg IV followed by maintenance doses of 7.5 mg/kg IV or orally every 6 hours[8]. For pediatric patients 3 months of age and older, meropenem dosing is based on weight and type of infection, with 10-40 mg/kg every 8 hours (maximum 2 grams every 8 hours)[10]. This combination therapy represents an important treatment option for complicated intra-abdominal infections, particularly in the context of increasing antimicrobial resistance.
