**MESA** is a multi-agent antineoplastic chemotherapy regimen comprising methotrexate, etoposide, dexamethasone, and pegaspargase, a pegylated L-asparaginase. It has been studied with radiotherapy for early-stage extranodal natural killer/T-cell lymphoma, nasal type. Methotrexate inhibits dihydrofolate reductase and folate-dependent nucleotide synthesis; etoposide poisons DNA topoisomerase II; dexamethasone activates the glucocorticoid receptor; and pegaspargase depletes circulating L-asparagine, exploiting lymphoma-cell dependence on extracellular asparagine.
