Mesdopetam is a novel small molecule dopamine D3 receptor antagonist with a strong preference for the D3 receptor over the D2 receptor. It is being developed primarily for the treatment of levodopa-induced dyskinesia (LID) in patients with Parkinson's disease and is also under investigation as a potential antipsychotic for Parkinson’s disease psychosis. Mesdopetam displays an agonist-like binding mode at the D3 receptor but lacks intrinsic activity, aiming to provide antidyskinetic and antipsychotic effects with fewer motor side effects compared to conventional dopamine antagonists. The drug has also shown some affinity for sigma-1 receptors and certain serotonin receptors (5-HT1A and 5-HT2A). Developed by IRLAB Therapeutics, mesdopetam has demonstrated safety and tolerability in phase 1 studies and efficacy in phase 2 trials[1][3][4][5][6].
