Mesenchymal precursor cell-expanded cord blood is an ex vivo expanded hematopoietic stem cell (HSC) therapy developed by Mesoblast for use in patients undergoing hematopoietic stem cell transplantation (HSCT). The therapy is produced by co-culturing a single unit of umbilical cord blood (UCB) with allogeneic mesenchymal precursor cells (MPCs), specifically STRO-3+ cells, which serve as a feeder layer. This proprietary expansion process significantly increases the number of CD34+ hematopoietic stem and progenitor cells (HSPCs) by a median factor of 30-49, addressing the critical limitation of low cell doses in traditional cord blood transplants. In clinical practice, the expanded unit is typically administered intravenously alongside an unmanipulated cord blood unit (a double cord blood transplant) to accelerate neutrophil and platelet engraftment and reduce the risk of infection and transplant-related mortality in patients with hematologic malignancies.
