Mesenchymal stem cell-derived exosomes are nano-sized extracellular vesicles (30–150 nm) secreted by mesenchymal stem cells (MSCs) that function as potent mediators of intercellular communication. These vesicles carry a complex cargo of bioactive molecules, including growth factors, cytokines, and microRNAs, which modulate the microenvironment of recipient cells. In the context of dermatology and aesthetic medicine, MSC-derived exosomes are utilized for their regenerative and anti-inflammatory properties. When administered intradermally, they stimulate dermal fibroblasts to enhance the synthesis of structural proteins such as Type I collagen and elastin, while simultaneously inhibiting the expression of matrix metalloproteinases that degrade the extracellular matrix. This specific therapy is currently being evaluated in a Phase 3 clinical trial (NCT05659030) led by investigator Mert Ersan to assess its efficacy in improving facial skin quality and reversing signs of aging, such as wrinkles and loss of elasticity.
