Overview

Mesenchymal stem cell (MSC)-derived exosomes are an experimental cell-free therapeutic being investigated by Mustafa Azizoğlu for the treatment of pilonidal sinus disease, particularly in pediatric patients. These exosomes are extracellular vesicles isolated from the conditioned medium of MSCs and processed into a lyophilized powder for topical application. They function by delivering a cargo of bioactive factors, including growth factors and cytokines, which facilitate tissue repair and modulate inflammation via paracrine mechanisms. In clinical trials, these exosomes are applied to the pilonidal sinus wound area, often in combination with crystallized phenol (a chemical sclerosing agent) or MSCs themselves, to accelerate the healing of chronic tracts and improve clinical outcomes compared to standard treatments.
