Mesenchymal stem cell exosomes (MSC-Exos) are nanoscale extracellular vesicles secreted by mesenchymal stem cells. They function as intercellular messengers, transporting proteins, lipids, nucleic acids (including microRNAs), and other bioactive molecules to recipient cells. These exosomes exhibit low toxicity and immunogenicity, high biocompatibility, and the ability to cross biological barriers such as the blood-brain barrier. Their natural regenerative properties mirror those of their parent MSCs but without the risks associated with live cell transplantation. MSC-Exos have demonstrated anti-inflammatory, anti-fibrotic, pro-angiogenic effects and tissue regenerative potential in preclinical models of cardiovascular disease, neurological disorders, kidney injury, fibrotic diseases, and cancer. They are also being explored as drug delivery vehicles due to their capacity for targeted delivery of therapeutic agents—including small molecules and RNA-based drugs—while minimizing off-target effects[1][2][3][5]. No FDA-approved or China NMPA-approved products currently exist; all clinical use is investigational[2].
