Mesenchymal stromal cells + vedolizumab is an investigational combination therapy comprising two distinct modalities: - **Mesenchymal stromal cells (MSCs):** Multipotent, immunomodulatory stem/stromal cells capable of modulating immune responses, promoting tissue repair, and restoring the intestinal mucosal barrier. MSCs exert their effects through paracrine signaling, secretion of bioactive factors (including cytokines and growth factors), cell-to-cell contact, and mitochondrial transfer. They are being studied for inflammatory bowel disease (IBD), Crohn’s disease, ulcerative colitis, and other immune-mediated conditions[9][3][8]. - **Vedolizumab:** A humanized monoclonal antibody that selectively binds to the α4β7 integrin on lymphocytes. By blocking this integrin’s interaction with mucosal addressin cell adhesion molecule-1 (MAdCAM-1) on gut endothelial cells, vedolizumab inhibits lymphocyte trafficking to the gastrointestinal tract and reduces gut inflammation. Vedolizumab is approved for moderate-to-severe ulcerative colitis and Crohn’s disease. The rationale for combining MSCs with vedolizumab is to leverage complementary mechanisms—MSCs’ broad immunomodulation/tissue repair with vedolizumab’s targeted inhibition of gut-specific lymphocyte migration—to enhance efficacy in refractory IBD or related conditions[4][7]. This combination remains experimental; clinical data are limited but suggest potential benefit in select patient populations.
