Overview

This therapeutic approach involves the co-transplantation of human mesenchymal stromal cells (hMSCs) and umbilical cord blood (UCB) directly into the bone marrow (intra-osseous) following reduced-intensity conditioning. Developed by the Case Comprehensive Cancer Center, the strategy aims to enhance the engraftment of hematopoietic stem cells in patients with various high-risk hematologic malignancies, including leukemias, lymphomas, and myelodysplastic syndromes. Mesenchymal stromal cells are multipotent cells that provide a supportive microenvironment (niche) for hematopoietic stem cells by secreting essential cytokines and growth factors and providing structural support through cell-to-cell contact. The intra-osseous administration route is specifically designed to bypass the systemic circulation and the 'homing' phase required during traditional intravenous administration, potentially leading to faster and more reliable neutrophil and platelet recovery in patients who might otherwise face delayed or failed engraftment.
