meso-4-1BB-CD3ζ-αPD1 CAR T cells is an autologous chimeric antigen receptor (CAR) T-cell therapy engineered to target mesothelin (MSLN) while simultaneously providing PD-1 blockade. Developed by Shanghai Tenth People's Hospital, this construct is designed as an "armored" CAR-T cell therapy to enhance efficacy in the immunosuppressive tumor microenvironment of solid tumors. The CAR construct incorporates a 4-1BB (CD137) costimulatory domain and a CD3ζ signaling domain for T-cell activation and persistence. The αPD1 modification (which may involve PD-1 knockout or the expression of an anti-PD-1 single-chain variable fragment) is intended to prevent T-cell exhaustion by inhibiting the PD-1/PD-L1 checkpoint pathway. The therapy is primarily being evaluated in clinical trials for recurrent or refractory ovarian cancer, often in combination with the multi-target tyrosine kinase inhibitor anlotinib to improve clinical outcomes.
