Mesocarb, also known by the brand name Sydnocarb, is a synthetic psychostimulant medication developed in the Soviet Union in the 1970s. It acts as a selective dopamine reuptake inhibitor (DRI), specifically functioning as a non-competitive inhibitor and negative allosteric modulator of the dopamine transporter (DAT). This mechanism increases synaptic dopamine concentrations, leading to psychostimulant effects with a slower onset and longer duration of action, and potentially lower abuse liability compared to traditional stimulants like amphetamines. It has been used in Russia and Eastern European countries for various psychiatric disorders, including asthenia, apathy, adynamia, depression, hyperactivity, and as an adjunct to levodopa therapy for Parkinson's disease. It has also been studied for its wakefulness-promoting and cognitive-enhancing properties. In the United States, mesocarb is a DEA Schedule I controlled substance due to its high potential for abuse and lack of accepted medical use.
