Mesoridazine is a phenothiazine antipsychotic, structurally related to thioridazine and classified as a typical antipsychotic. It was primarily used for the management of schizophrenia, especially in patients who did not respond adequately to other antipsychotics. Its mechanism of action involves antagonism at central dopamine receptors (mainly D2), leading to reduced dopaminergic neurotransmission in the brain, which helps alleviate psychotic symptoms such as hallucinations and delusions. Mesoridazine also exhibits central antiadrenergic, antidopaminergic, antiserotonergic, and weak muscarinic anticholinergic effects. The drug was withdrawn from the US market due to its association with life-threatening cardiac arrhythmias (QT prolongation) and sudden death[1][3][5]. It is an active metabolite of thioridazine.
