Mesothelin CAR T cells are a form of cell therapy in which human T lymphocytes are genetically engineered to express a chimeric antigen receptor (CAR) that specifically targets mesothelin, a cell-surface glycoprotein highly expressed on various solid tumors, including mesothelioma, ovarian, pancreatic, and some lung cancers, but with limited expression in normal tissue[1][3][4]. These CARs often incorporate a single chain variable fragment (scFv) that recognizes mesothelin, coupled with intracellular co-stimulatory and activation domains (commonly CD28 or 4-1BB with CD3ζ), enhancing T cell proliferation, persistence, activation, and cytotoxicity against mesothelin-expressing tumor cells[1][3][4]. Clinical studies have explored intravenous, intratumoral, and locoregional administration in solid tumors, and efforts are ongoing to improve persistence and efficacy by optimizing the CAR design and administration route[3][4]. The therapy is in various stages of clinical and preclinical development; no marketed product currently exists[2][3][4].
