Mesothelin-targeted CAR-T cells are an autologous cell therapy consisting of T cells genetically engineered to express a chimeric antigen receptor (CAR) specific for mesothelin (MSLN). Mesothelin is a cell-surface glycoprotein that is highly expressed in several solid tumors, including malignant mesothelioma, pancreatic adenocarcinoma, and ovarian cancer, while showing limited expression in normal tissues. Upon infusion, these CAR-T cells recognize and bind to mesothelin-expressing tumor cells, leading to T-cell activation, expansion, and targeted cytotoxicity. Various iterations of these cells are being investigated in clinical trials, including those developed by Jiangsu Provincial Hospital, often incorporating co-stimulatory domains like 4-1BB or CD28 to enhance persistence and efficacy in the immunosuppressive solid tumor microenvironment.
