metamizole

Metamizole (also known as dipyrone) is a non-opioid analgesic and antipyretic small molecule belonging to the pyrazolone family. It is used for the treatment of severe pain and fever, often when other treatments are contraindicated or ineffective. Its mechanism of action is complex and involves the inhibition of central prostaglandin synthesis (potentially via COX-3 inhibition), activation of the endogenous cannabinoid system (CB1 and CB2 receptors), and modulation of the endogenous opioidergic system. Boehringer Ingelheim has investigated a new oral pharmaceutical form of metamizole specifically for its efficacy in reducing morphine requirements in patients with moderate to severe chronic cancer pain (neoplastic pain). Despite its widespread use in many regions, metamizole is restricted or banned in several countries, including the United States, due to the rare but serious risk of agranulocytosis.
